    In the opening paragraph at the top of the page is a link that says quite clearly “handy little form” and instructs quite clearly that it is the link to the form. Put your curser on it and left click. DUH!
    Would love to win this one, My daughter does all of her shopping at Macy’s and she introduced me to Charles David and now that I can shop online (I am handicapped) I can buy from all my favorite designers and CD is certainly high on my list.
